Effective January 1st 2006, all requests for increases in
coverage, and all new applications for coverage must include a
properly completed Building Valuation Worksheet from the MN FAIR
Plan MSB RCT Express web site. To
use this new evaluation service, you must first sign into our
secured site using your MN FAIR
Plan user id and password.
If you are a new agent to the MN FAIR Plan, you must
first submit an IRS W-9 form in order to receive an agent
number, and proceed into the secure site. Please complete
the W-9 form and submit it to
the MN FAIR Plan via fax at 612-338-4543, attn:
Accounting Department. Please include your e-mail address
and telephone number. Once the accounting department has
completed the process, you will be contacted with a user ID and
password, enabling you to log in and begin using the secured
site. |
